Census Form '06

Our last parish survey was over twenty years ago and our records need updating, particularly concerning the children of the parish.

Above you will find the census form available for download. Please fill it out and return it the rectory office. Please print all information. A yes or no is sufficient for first communion and confirmation information.

Please list all children presently living at your home or attending college. If an adult son or daughter living at home (and not a student) is not receiving a set of envelopes, they should also register. That information can be written on a separate sheet of paper and returned with this form.

Please mail these census sheets to the rectory or drop them in the collection basket at Sunday Mass.

As we are transferring information to a new software package, we are hoping to have all information back to the rectory by mid May.
